How Do I Decide Whether To Have A Cataract Surgery? How Do I Choose Whether I Should Get a Cataract Surgery? Tell your Doctor how your cataract affects your vision & your life. You and your Doctor can decide accordingly. I do not see well enough to do my best at work I do not see well enough to do things I need to do at home. I do not see well enough to do things I like to do. (E.g. T.V. sw, play cards, go out with friends.) I am afraid I will bump into something or fall. I am not as independent as I was earlier. My glasses do not help me see clearly. Driving is a problem at night and there is too much glare from headlights. Is your eyesight getting worse? - Ojaseyehospital

If you feel your eyesight is getting worse either suddenly or gradually, the best thing would be to visit an ophthalmologist (eye doctor). To understand why your eyesight may be deteriorating, we take a closer look at a few reasons: Younger age groups : (upto 40 years ) : Refractive errors: This is the most common cause of defective vision in this age group. It can either be for distant objects (Myopia) or for near (Hypermetropia). This can very easily be corrected by either glasses or contact lenses. Surgical procedures like LASIK, Contoura, and Phakic Intraocular lenses are other options that can free you from spectacle dependency. Keratoconus: This is a condition in which the front portion of the eye (i.e cornea) bulges forward resulting in blurring of vision, double vision, frequent changes of glass power, and poor quality of vision even after wearing spectacles. Various modalities are available for treatment including collagen cross-linking with riboflavin (C3R), INTACS, and customized contact lenses. Spasm of accommodation: It is seen in individuals involved in prolonged near work could be reading, computer use, etc. In this condition following prolonged near work, the distance vision appears blurred. Pt also complains of tired eyes, frequent headaches, and eye strain while working. The best treatment is to take a break from work for a day or two to let your eye muscles, the much-needed break. Computer vision syndrome: Eye strain, headache, neck pain, watering, blurring of vision, dry eyes experienced due to prolonged hours spent using visual devices like computers, phones, tablets, television, etc is known as computer vision syndrome. Taking frequent breaks, good work posture, blinking eyes consciously, and maintaining adequate lighting will help reduce the symptoms. Central Serous Retinopathy (CSR): CSR is a build-up of fluid under the retina resulting in diminished or distorted vision. Usually affects one eye at a time, but rarely can affect both eyes. It is most commonly seen in males in the age group of 30 – 50 years. Stress is a very important risk factor. Usually, it resolves in one to two months without any treatment with good recovery of the vision. Middle age group (45 to 60 years ) : Presbyopia : As you age, especially in the 40 to 50 age bracket, your eyesight may decline for close-up tasks, such as reading and looking at the fine print of smartphone screens. This happens because your crystalline lens becomes less flexible, making it hard to focus on close-up images. This condition is called presbyopia and will affect all of us at some stage in our lives. This is very easily corrected with glasses. Glaucoma : It is a group of conditions in which the nerve connecting the eye and the brain (optic nerve ) gets damaged . Most common cause is rise in intra-ocular pressure (eye pressure). The more common type of glaucoma (open angle )often has no symptoms other than very slow vision loss. Hence it is very important to get your eyes checked routinely especially if you have any family members who suffer from glaucoma. Main treatment consists of antiglaucoma eyedrops. Surgery is considered when there is no improvement with medication. The other type of glaucoma (angle closure) is a medical emergency and is characterised by very painful, watery eyes, photophobia and blurring of vision. Immediate medical help should be sought. Cataract : Many people especially in their late 50s or early 60s start to develop cataract. Symptoms include changing glass power, blurring vision, difficulty in night driving , glare etc. A visit to your ophthalmologist will help clarify whether you have this problem or not. Treatment consists of surgical removal of the cataractous lens followed by implantation of artificial lens inside the eye. Diabetic Retinopathy : It is caused due to damage to the blood vessels of the retina due to poorly controlled blood sugar. A yearly check- up of retina by an ophthalmologist is a must to detect this problem. Depending on the condition, treatment consists of laser to the retina and injections given into the eye. In very advanced cases surgical intervention may be necessary. Older age group (Above 60 years) : Cataract : It is the most common cause for reducing vision in this age group.Symptoms include changing glass power, blurring vision, difficulty in night driving, glare etc. A visit to your ophthalmologist will help clarify whether you have this problem or not.Treatment consists of surgical removal of the cataractous lens followed by implantation of artificial lens inside the eye. Glaucoma : It is a group of conditions in which the nerve connecting the eye and the brain (optic nerve ) gets damaged . Most common cause is rise in intra-ocular pressure (eye pressure). Surgery is considered when there is no improvement with medication The other type of glaucoma (angle closure) is a medical emergency and is characterized by very painful, watery eyes, photophobia, and blurring of vision. Immediate medical help should be sought. Diabetic Retinopathy: It is caused due to damage to the blood vessels of the retina due to poorly controlled blood sugar. A yearly check-up of the retina by an ophthalmologist is a must to detect this problem at the earliest and to provide necessary treatment to prevent permanent vision loss. . Depending on the condition, treatment consists of laser to the retina and injections given into the eye. In very advanced cases surgical management may be necessary. Age-Related Macular Degeneration (ARMD): It is degeneration of the retina which affects the central vision. It could either be dry ARMD where the central retina (macula) deteriorates or wet ARMD where leaky blood vessels grow under the retina. A special combination of vitamins and minerals may reduce disease progression in dry ARMD. Whereas LASER and eye injections are used to treat wet ARMD. Cataract Surgery in Mumbai | Cataract Treatment in Mumbai - Ojas Eye Hospital

The opacification of the normal transparent lens is called cataract. The Latin word ‘cataracta’ means ‘waterfall’. Imagine trying to peer through a sheet of falling water or through a frosted or fogged-up window. Development of Cataract varies from person to person but as a general rule, most cataracts develop slowly over a period of time. A cataract can take months or even years to reach a point where it adversely affects vision. Causes of cataract Age-related cataract The cataract occurs as a result of the natural aging process of lens fibres which become opaque over a period of time. Traumatic cataract Due to a direct penetrating injury.Concussion- Electric shock and lightning.Ionizing radiation done as treatment for ocular tumors.Surgical trauma Metabolic cataract: Defect in body metabolism DiabetesGalactosaemia -inborn error of metabolismCalcium disorders Steroid-induced cataract This occurs as a result of excess intake of oral steroid or putting steroid drops in the eye. Secondary cataract Here, cataract develops as a result of some other primary ocular disease such as chronic eye inflammation or glaucoma. History of Cataract treatment The earliest surgery treatment was started in India, by Maharshi Sushruta. It was known as ‘couching’, where the cataractous lens was dislocated backward into the bottom of the eye and out of visual axis. This procedure was performed for more than two thousand years until the mid-eighteenth century. Great progress in cataract surgery has been made in recent years with the introduction of micro-surgical instruments, microscope and modern surgical techniques like phacoemulsification, which has made couching obsolete. In the early stages of cataract development, all that is needed to correct your vision with glasses is a change in prescription. As the cataract develops and begins to affect your lifestyle, it needs to be removed. Cataract surgery, the most commonly performed operation, is safe and effective in 95% patients with enhancement in vision. Methods of evaluation of cataracts: Visual acuity: Checking vision of both eyes unaided and aided with glasses and pin-hole vision to know the improvement as well as to get the general idea about the macular function of the eyes. This will help in prognostic evaluation of visual recovery after cataract surgery. Intra ocular pressure: If intra-ocular pressure increases as a secondary to cataract surgery is needed to prevent further complications. Slit-lamp examination: To know the type of cataract along with its opacity, morphology and etiology or any associated ocular pathology. Direct and indirect ophthalmoscopy: for complete retinal evaluation. Dense cataract will prevent retinal evaluation and such cases need B-scan for retinal evaluation. A-scan biometry: To calculate the AL and IOL power for implantation in cases of mature cataract the posterior segment of the eye is evaluated. Optical biometry: this is an advanced non contact method to measure IOL power. It is patient friendly and highly precise Treatment Options Available For Cataract Extra Capsular Cataract Extraction (ECCE): is a conventional technique. ECCE requires an incision of 10-12mmThe doctor removes the clouded lens in one piece.Multiple stitches are required.Doctor implants a non-foldable lens. Phacoemulsification – A Micro Incision Cataract Surgery Phacoemulsification is latest technology in Cataract surgery. It is a micro-incisional stitch less operation where cataract is emulsified by ultra sound energy, liquefied & sucked through the phacoemulsifier probe. A foldable intra- ocular lens is then implanted in the eye permanently. Vision restoration is possible in a short period of time & is least traumatic with early rehabilitation & recuperation. Advantages It is Stitchless. Hence healing is very fast. It is done under Topical anaesthesia (by putting drops.) So no need to patch the eye except in special circumstances Vision correction for far & near is possible Patient can get back to work in the shortest possible time Lens implants During cataract surgery, the natural lens of the eye that has turned opaque is removed, resulting in loss of focusing power of the eye. This situation would be parallel to clicking a photograph without a camera lens–the picture would be extremely blurred. When the natural lens of the eye is removed an artificial implant is placed in the eye. Intraocular Lens An intraocular lens (IOL) implant is a synthetic, artificial lens placed inside the eye that replaces the natural lens which is surgically removed usually as a part of cataract surgery. Monofocal intraocular lens can be used to give clear point focus either at a distance or close up, but one can choose only one focal point. Multifocal IOLs are popular as they allow correction of vision for both far & near distance. Trifocal IOLs provides excellent vision not only for near and far, but also for intermediate distances, especially for people who have routine usage of computers. Apodized Diffractive Multifocal IOL Gradual diffractive steps on the IOL implant that create a smooth transition between focal points. The IOL bends incoming light to the multiple focal points to increase vision in differing light situations. Accomodative IOL Crystalens and Trulign Toric currently are the only US-FDA -approved intraocular lenses (IOLs) that use a method called accommodation, enabling sharper vision at multiple distances for people who have undergone cataract surgery. An accommodating IOL shifts position with the action of eye muscles and movement to improve eyesight. Toric IOL for astigmatism This is a monofocal IOL with astigmatism correction built into the lens. Astigmatism: This eye condition distorts or blurs the ability to see both near and distant objects. With astigmatism the cornea (the clear front window of the eye) is not round and smooth (like a basketball), but instead is curved like a football. People with significant degrees of astigmatism are usually most satisfied with toric IOLs. People who want to reduce (or possibly eliminate) the need for eyeglasses may opt for an additional treatment called limbal relaxing incisions, which may be done at the same time as cataract surgery or separately. These small incisions allow the cornea’s shape to be rounder or more symmetrical. Protective IOL filters IOLs include filters to protect the eye’s retina from exposure to UV and other potentially damaging light radiation. The eye doctor selects the filters that will provide appropriate protection for the patient’s specific needs. Other important cataract lens replacement considerations In some cases, after healing completely from the cataract lens surgery, some people may need further correction to achieve the best vision possible. Their ophthalmologist may recommend additional surgery to exchange an IOL for another type, implant an additional IOL, or make limbal relaxing incisions in the cornea. Other laser refractive surgery may be recommended in some cases. People who have had refractive surgery such as LASIK need to be carefully evaluated before getting IOLs because the ability to calculate the correct IOL prescription may be affected by the previous refractive surgery. #cataractsurgeryinmumbai #eyehospitalinmumbai For More Information- https://www.ojaseyehospital.com/ About Doctor- https://medicalcaree.blogspot.com/p/eye-specialist.html

Like a camera, the eye sees when light enters it. Inside your eye is a natural clear lens, much like the lens of a camera. It focuses the light rays coming through the pupil onto the retina at the back of the eye. The different parts of the retina collect this light and send a message to your brain, enabling you to see. For perfect vision the lens should be clear so that light can pass through it. Light enters through the cornea, passes through your natural lens and is focused onto your retina, resulting in clear vision When the lens becomes cloudy or opaque, light cannot pass through it. Cataract scatters the light rays passing through the eye, thus resulting in blurred vision. A cataract is a condition when the natural lens inside your eyes gets cloudy. Compared to vision with a clear lens, your vision with a cataract may look fuzzy, colors might be muted, and you may experience difficulty in reading signs while driving at night. The symptoms of cataracts are progressive and cannot be corrected with glasses or contacts. When symptoms become bothersome or limit your daily activities, it’s time to consider treatment. What causes cataract? The most common cause of cataract is degenerative changes due to the ageing process. With age, proteins in the natural lens degenerate, resulting in a clouded lens called a cataract. There might be other causes such as: Health conditions like diabetes, kidney disease, glaucoma, smoking, eye injuries, infection, and inflammation inside the eye Prolonged use of certain medications can also lead to cataract formation Cataract may also occur in children due to genetic or metabolic defect or due to infection and trauma What are the symptoms of cataract? If you have cataract, you may experience some or all of the following symptoms. Cloudy vision Colors of objects may appear faded Poor eyesight at night Difficulty in driving at night, especially because of the glare of lights Difficulty in reading in dim light Colored haloes You may see multiple images or double images How can the cataract be removed? In cataract surgery your natural clouded lens will be replaced by an artificial intraocular lens (IOL). This lens helps the incoming light rays to focus properly on your retina, thus enabling you to see clearly. Your new lens should be able to restore vision to nearly what it was earlier, though you may need to wear glasses for reading or driving. The lens options include MONOFOCAL lenses – corrects distance vision, and bifocal lens – corrects distance and near vision and trifocal lens corrects distance, intermediate and near vision. In some cases when the shape of the eye is not circular resulting in distorted vision – a condition called as astigmatism, an option of customized “toricmonofocal lenses” or “toric multifocal lenses” can be used. What surgical techniques are available at ojas to remove the cataract? OJas offers various advanced, cutting edge surgical techniques to treat cataract. Based on the type of cataract you have, your health condition and your lifestyle, the Ophthalmologist will recommend the best suitable option for you. The three commonly used surgical techniques are:- Small Incision Cataract Surgery (SICS): This is also a new technique where the cataract is removed manually through a slightly larger incision. This technique is used if your cataract is excessively hard, making phacoemulsification difficult. This is rarely used Phacoemulsification: In this method a tiny instrument is inserted through a small incision – (approximately 2.2 – 2.8 mm wide). Ultrasound vibrations break the cataract into tiny pieces. These pieces are gently suctioned out by vacuum. Usually, no stitches are required to close the incision. This minimally invasive surgery allows faster and safer healing, and you can return to your normal activities in no time. Microincision Cataract Surgery ( MICS) Phacoemulsification is carried out through an extremely small incision sized 1.8 mm. The primary advantage is extremely reliable and predictable outcomes with fast healing. Understanding Cataract with Dr. Niteen Dedhia | Cataract specialist in Mumbai | Cataract surgery in Mumbai. India

Cataract surgery in Mumbai. India

Learn more about Cataract, Symptoms of Cataract, Treatment of Cataract, Misconceptions about Cataract, and Cataract operations. --------------------------- I am Dr. Niteen Dedhia I am Ophthalmologist and Medical director of Ojas Eye Hospital & we have two centres in Mumbai one in Bandra and one in Kandivali. Understanding Cataract with Dr. Niteen Dedhia The most common cause of cataract is age. Some of the other cause of cataract are diabetes, Injuries to eye and thyroid disorder. Symptoms of Cataract The Symptoms related to Cataract are the patients normally see the blurred vision; he has difficulty in Night Driving and also sometimes as Double vision. All this condition can occurs after the age of 40 and so we advices most of the people to undergo or to have eye examination with an Eye Doctor as soon as they have problem related to vision problem. Treatment of Cataract Cataract surgery evolved in the last 2 to 3 decades. Initially when we used to perform cataract operation the whole cataract was removed from within the eye through a very large incision which is almost about 8 to 10 mm inside. After removing the cataract, we were taking stitches to close the incision and a riches lens was implanted in India. Now the technology as evolved and performed the cataract operation just 2 mm incision and this incision is also made either with laser or with diamond instrument. Once cataract is emulsified or liquefied inside the eyes. It is sub out and it become almost like water after removing this cataract the lens implanted in the eye from this lens is special foldable lens and it passes through the same 2 mile meter incision and it unfold him inside the eye. There are different types of lens which are available: They could either unifocal lens which allows you to see very fear very clearly and for reading you have to reading glasses. The Other lens which is available is multifocal lens and did in multifocal lens we have byfocal and trifocal lens and they give you good crystal vision work for far, near as well intermediate vision. Misconception of Cataract There are lots of Misconception of about Cataract operation previously patients they use to undergo cataract operation only when cataract was right or when it is mutual but now a day’s cataract operation can be done even if its just 10 percent or 20 percent or when the patient starts having Difficulty in Night driving, or safe thing. The kind of number you get after the cataract operation is also very mineable Because we are able to remove the power of the glasses once cataract is implanted This is because cataract operation as it is done the lens which is implanted is of specific power that power point size the glasses power so its help one to remove the glasses look booth for far as well as near if we are implanting the lens which is the multifocal lens.
